Telephone Reassurance

Provides regular contact and safety check to reassure and support senior citizens who are homebound.

A telephone call is made to seniors each weekday to check on their well being, and any concerns are referred to appropriate family members, friends or the elderly’s health team members, or other agencies.  Early intervention strategies such as this one may delay or prevent nursing home placement, which is more costly than community-based services.  In Wyoming, the average medical assistance payment for one year in a nursing home is over $37,000.00.

Telephone reassurance is also a useful tool to assist in care transitions to ensure the senior’s transition care plan is being followed to prevent problems and possible re-hospitalization.
